A cost-effective plan that focuses on preventive care. Services must be provided by a participating Delta Dental PPO Network dentist. It pays up to $1,000 per person each calendar year, with 100% coverage for preventive services and no deductible.

This plan offers enhanced coverage for a wider range of services. It provides up to $1,500 per person per year after a $50 deductible (3x family maximum) and includes 100% preventive, 70% basic, and 50% major services after a one-year waiting period. It includes PPO Preferred and Premier network options.

Our most comprehensive plan, offering the highest annual maximum. It allows up to $2,000 per person each calendar year after a $50 deductible (3x family maximum) and provides 100% preventive, 80% basic, and 50% major services. It also includes 50% orthodontia for dependent children up to a lifetime maximum of $1,000. It includes PPO Preferred and Premier network options.
